Zencora Neuro Sound and Vibroacoustic Experiences Waiver

1. Nature of Zencora Neuro Experience

Zencora Neuro Sound and Vibroacoustic Experiences use low-frequency sound vibrations,music, and sensory-reduced environments to foster relaxation and enhance well-being. These experiences are delivered through specialized music furniture that creates an immersive environment through sound, vibration, and potentially visual elements.

2. Non-Medical Experience

I understand that Zencora Neuro Experiences are not a substitute for medical treatment. The practitioners are not licensed healthcare professionals, and these experiences are complementary to conventional wellness practices. They are not intended to diagnose or treat any medical condition.

3. Potential Risks and Contraindications

While these experiences are generally safe, certain conditions require consideration and consultation with a healthcare professional before participation, including:

  • Epilepsy or seizure disorders

  • Heart conditions (including pacemakers or other implanted devices)

  • Recent surgeries

  • Severe migraines

  • Mental health conditions

  • Pregnancy

  • Vestibular conditions

  • Cochlear implants or hearing aids

It is the participant's (or in the case of minors, the parent/guardian's) responsibility to ensure that the participant is medically fit to participate.

4. Flashing and Repetitive Lights, Audio, and Vibrations

During Zencora Neuro Experiences, flashing and/or repetitive lights, audio patterns, and vibrations may be used, with or without screens, projectors, or virtual reality (VR). As with video games or other sensory sequences, care should be taken, especially for individuals with a history of epilepsy, sensitivity to visual stimuli, or vestibular issues. If concerns exist about sensitivity to flashing lights, repetitive audio sequences, or vibrations, participants are encouraged to consult a medical professional before participating.

5. Consent to Participate

By signing below, I consent to participate (or allow the minor to participate) in the Zencora Neuro Experience, which may include sound, vibration, and visual elements such as flashing or repetitive lights. I understand that this experience is intended to promote relaxation and well-being but is not medical in nature.

6. Participant Responsibility

I agree to inform the practitioner of any discomfort experienced during the session. I acknowledge that I am (or, in the case of minors, the parent/guardian is) responsible for my (or the minor's) well-being before, during, and after the experience.

7. Assumption of Risk

I voluntarily assume all risks associated with the Zencora Neuro experience. I understand that there may be unknown risks, particularly for individuals with certain medical conditions. I confirm that I have consulted a healthcare provider, or I have made an informed decision to proceed without such consultation.

8. Release of Liability

I hereby release SpaDelivered LLC, its practitioners, and affiliates (including but not limited to Canvas Counseling & Wellness, PLLC) from any liability, claims, or demands related to injuries, adverse effects, or complications arising from my (or the minor's) participation in the Zencora Neuro experience.

9. Cancellation Policy

Cancellations must be made at least 24 hours in advance. Failure to provide notice may result in a full service fee being charged.
